A lot of ISPs set up transparent web proxies between their users and the outside world, and quite often the proxies' cache gets stale. I've seen this happen a lot with people's avatars. As Mega and RJ said, it was because the file was cached. Also, I get the same problem with Postline links being opened with the default mahogany backdrop.
